Why should you take a

Community Health Survey?

Many factors influence health and well-being in a community, and many entities and individuals in the community have a role to play in responding to community health needs.

Healthy Hometown Coalition sees a requirement for a framework within which a community can take a comprehensive approach to maintaining and improving health: assessing health needs, determining resources and assets for promoting health, developing and implementing a strategy for action, and establishing where responsibility should lie for specific results.

Healthy Hometown Coalition proposes a community health improvement process as a basis for community collaboration in monitoring overall health matters and in addressing specific health issues. This process can support the development of shared community goals for health improvement and the implementation of a planned and integrated approach for achieving those goals.

The Clinton County Healthy Hometown project is devoted to identifying health challenges, improving health services, reducing health risks, and promoting health equity. In the initial phase, we will be collecting data and developing ideas for a strategic plan for our community.
