, 55, passed away Sunday, June 21, 2020, at Nortons Hospital in Louisville, Kentucky.

He was the son of Tommy Ray Blevens and the late Amdana Cook Blevens. In addition to his mother, he was preceded in death by a brother, James Shelton.

He survived by his children,T ommy Blevens III, Adam Blevens, Michael Polston, Brianna Polston and Cody Blevens; his girlfriend, Debra Polston; his siblings, Ronnie Shelton, Donnie Blevens, Diane Shelton, Bobby Blevens, Linda Moore, Carol Looper and Sandra Garner; nine grandchildren.

Services were held Thursday, June 25, at the Weldon Haddix Funeral Home with Bobby Brown officiating. Burial was in the Hopkins Cemetery.

Arrangements were by the Weldon Haddix Funeral Home of Albany.
